As a community foundation comprised of more than 2,000 charitable funds, the New York Community Trust makes grants within the five boroughs of New York in four program areas: Community Development and the Environment; Health and People with Special Needs; Education, Arts, and Human Justice; and Children, Youth, and Families.
Funding from the New York Community Trust supported the development of a new advanced role for home care workers focused on improving care transitions. Implementation of eight full-time Care Connections Senior Aides over 18 months in 2015 and 2016 contributed to reduced ER admission rates among more than 1,400 home care clients.
