PACE Central New York (see case study) is a managed long-term care program serving annually over 400 nursing home eligible seniors.
Concerned about high turnover rates among new home health aides, PACE partnered with PHI to assist in designing and implementing two programs that would improve retention of their geriatric care workers (GCWs): peer mentoring and an advanced GCW II role.
