SUMMARY: To improve resident and employee experience, as well as resident outcomes, Orchard Cove launched a number of culture change initiatives in 2005.

With technical support and training received through the PHI Center for Coaching Supervision and Leadership, the Orchard Cove leadership transitioned the skilled-nursing facility to a household model; improved staff satisfaction; and dramatically decreased the incidences of pressure ulcers, urinary tract infections, falls, and depression among residents.

Employer Outcomes From January 2008 to June 2010, the rolling average of CNA call-outs fell threefold
Employee Outcomes 87% of the nursing staff agrees with the statement: “My supervisor informs me about issues that are important.”
Resident Outcomes The percentage of high-risk residents with pressure ulcers zeroed out beginning in March 2010
The percentage of residents with urinary tract infections exhibited a downward trend starting in June 2008, reaching zero in July 2010
