Cean comes to PHI with over a decade of experience in deep cultural transformation in long term care. She has worked with a range of long-term care organizations throughout the Midwest in implementing person-centered practices, transforming workplace culture, and improving quality of care, quality of life, and quality of jobs.

She is a certified Eden Alternative® educator and mentor, and has adapted the PHI Coaching Supervision and Self-Managed Work Teams curricula for Eden Registry Organizations. In her role as co-chair of the Michigan Alliance for Person Centered Communities, Cean is involved in spearheading a statewide culture change initiative involving stakeholders throughout the eldercare and disability services community — including state regulatory agencies — to address the barriers to implementing person-centered care in Michigan.

Cean is a frequent national speaker on topics related to organizational culture change. Before coming to PHI, Cean served as Vice President of Culture Transformation at Trinity Senior Living Communities (TSLC), where she lead the culture change initiative for this large, multi-state/multi-facility organization, and developed the TSLC Sanctuary model of care. She received her BS from Michigan State University.

As an Organizational Change Consultant, Anna supports long term care providers to create cultures of quality of care, quality of life, and quality of jobs.

Anna joins PHI after working in the fields of gerontology and long-term care for more than three decades. After beginning her career as a registered nurse, Anna has since focused her career on working with elders in a variety of settings, including nursing homes, adult day care, and home care settings.

Anna is President of the Board of the Pioneer Network, a Fellow of the American Academy of Nursing, and the primary author of LEAP for the 21st Century LTC Workforce. She has also served as Vice President of Culture Change at Life Services Network of Illinois, Director of Residential Care Services for the Rush Alzheimer’s Disease Center in Chicago, and Associate Director of Long Term Care Accreditation at the Joint Commission.

Anna spent the last six years at The Green House Project, where she worked closely with PHI to adapt the PHI Coaching ApproachSM to fit the Green House® model. She frequently appears at various national and international conferences, speaking about culture change, dementia, and other long-term care issues. A published author, Anna most recently contributed a chapter to the book Models and Pathways for Person-Centered Elder Care (Health Professions Press, 2013). Anna received her Master of Science in Nursing at Northern Illinois University.
