		<description>As a home care aide it is important to know what other members of the team are doing for the client. The aide is with the client the most and knows the needs but may not know how they are being addressed. It is especially difficult when there is a multidisciplinary team in the home which sometimes doesn&#039;t even communicate with each other, so that the aide may be getting mixed messages about how to do care. The aide may also rely on the client for information and direction. If the client is confused, that is really confusing for the aide.</description>
